Social Care and Wellbeing Hub

Description

This is one of three teams across Calderdale that can help you to regain mobility and independence at home and within your community. You can get support if you have had a stroke, a fall, or a period of illness, or if you have a chronic long term health condition such as multiple sclerosis or Parkinson's disease.

The team includes dietitians, physiotherapists, speech and language therapists, occupational therapists and podiatrists. You can be referred to the team by hospital or community based staff. A member of the team will visit you at home to assess your needs, discuss treatment and plan an individual rehabilitation package.

Due to high demand there may be a waiting list for this service.
