District nurses (Calderdale and Huddersfield NHS Foundation Trust)

Web: See external link policy

District nurses visit people in their own homes or in residential care homes, providing care for patients and emotional support for their families and carers.

District nurses can offer nursing care (such as wound dressing and giving injections), as well as palliative and end of life care. They can support and advise on health care and preventing ill health. They can provide equipment to help people live independently, and support people to recover from accidents, ill health or a stay in hospital.

You can contact your district nurse through your local health centre.
