Community matrons (Calderdale and Huddersfield NHS Foundation Trust)

Community matrons support people with serious long term conditions or a complex range of conditions. Examples include diabetes, multiple sclerosis, chronic obstructive pulmonary disease and other respiratory diseases, and heart disease. They specialise in helping people who have had several emergency hospital admissions or are at risk of having to move into a nursing home because of their condition.

Your GP can tell you whether you can get a community matron. A community matron will work closely with you and your GP to plan and organise your care. They will make sure you get the right care and medication, organise support to help you stay in your own home, and act as a single point of contact for support and advice.
