Better Care Fund - Calderdale Plan
The Better Care Fund (BCF) was introduced by Central Government to promote integration in health and social care, with the aim of providing better coordinated services for people needing support.
Background information about the policy is available on the Better Care Fund GOV.UK| website.
The Calderdale Plan for the Better Care Fund is not, however, just about health and social care: it brings together the wider services which can support people to live independently at home: the third sector, community groups, housing and civic sector. It is about communities and our society.
The Plan provides information about how the Better Care Fund will make a difference in Calderdale, its objectives and methodology. Calderdale received ‘Approved’ status from NHS England for their BCF Plan in August 2016 and is one of only five authorities across the Yorkshire and Humber region to receive this status.
