Individual Service Funds (ISF)

Information for organisations

The Council is seeking expressions of interest from suitable organisations (including small and medium sized enterprises, voluntary and community sector and similar organisations) to facilitate and/or provide a range of Services for adults with assessed, eligible social care needs via an Individual Service Fund.

Interested parties are asked to read the documents below and complete all the forms including the Quality Assurance & Safeguarding Questionnaire which will be evaluated on a pass or fail basis.

The successful organisations who qualify following assessment of the Quality Assurance & Safeguarding Questionnaire will be included on a nil value register of organisations, held by the Council, for an initial term up to 31 March 2017.

The register will be used by individuals to select an organisation(s) to help them to meet the outcomes identified in their support plan via an Individual Service Fund.  Organisations shortlisted by individuals may be required to complete further evaluation before being offered an Individual Service Fund contract with that individual.

All completed submissions (Quality Assurance and Safeguarding Questionnaire, Provider Profile and BACS Form) must be e-mailed to at the time of application. Without all these forms the Council will not be able to process your application and no further notice will be given. Any queries may also be directed to the same email address.
