Access to services for older people, people with physical and sensory disabilities and their carers
There are four teams across Calderdale that cover specific groups of GP practices. All of these teams are accessed via Gateway to Care on 0845 11 11 103. Our Wellbeing and Social Care Advisors can give information about all services provided by Adults, Health and Social Care.
The Gateway to Care Team can deal with many queries whilst you are on the phone, this includes access to community alarms, frozen meals, blue badge applications, bus passes for disabled people under 60 and small pieces of equipment. The Gateway to Care staff will also be able to give you information and advice and signpost you to other services as appropriate.
Anyone who contacts Gateway to Care is still entitled to a full assessment of their needs if they wish. These people will be referred to our specialist teams to complete a Community Care Assessment.
Our specialist teams will then deal with requests for services. Staff in these teams can give advice and support to older people and people with physical and sensory disabilities to enable them to live as independently as possible in their own homes or in suitable residential settings in the community.
Support is also available for people who are providing care for someone who is dependent upon them.
There is a charging system in place which is explained fully by staff to service users and their carers. Service users are also given full information about the Complaints and Compliments processes.
Access to health services
Gateway to Care staff can now also give you advice and help to get back your independence, mobility and confidence after an accident, illness, injury or deterioration in a condition.
Hospital Social Work Team
If you are going into hospital you may need help when you leave to continue to live in your home. If requested, a social worker from Adults, Health and Social Care will look into what help you need and decide with you what services can be provided.
You can either contact Gateway to Care before you go into hospital or you can ask to see a social worker during your stay in hospital.
