Help leaving hospital

Who we help

We help adults / older people with health-related needs, physical disabilities, mental illness or learning disabilities.

To ensure the best use of our resources we have eligibility bands and criteria for getting services. We will assess your needs and, if it is agreed that services are needed, a "support plan" will be drawn up for you.

The effect on carers of providing help is not taken for granted. Carers can ask for a Carer's assessment .
