Eligibility framework
Criteria: Moderate
When
- There is, or will be, an inability to carry out several personal care or domestic routines.
- Involvement in several aspects of work, education or learning cannot or will not be sustained.
- Several social support systems and relationships cannot or will not be sustained.
- Several family and other social roles and responsibilities cannot or will not be undertaken.
Criteria
- People who, because of their physical or mental state require or will require regular support or supervision to enable them to live independently in the community or who require only specialist equipment or adaptations.
- These people may only require care two or three times a week, but without this relationships, social responsibilities, work, education and social activities will be difficult to maintain.
Services which may be provided following assessment
- Personal care, day care, meals services, community alarm (where there is risk of falling or risk of crime)
- Specialist equipment or adaptations.
