Community care

Introduction

Community care is about enabling people who need care to live as independently as possible in their own homes or in suitable residential settings in the community. It includes support for people providing care for someone who is dependent on them.

Clear information

We will give information on all aspects of assessments and service provision ; such as how to ask for an assessment, or what is meant by a care plan.

We will explain in plain language how our assessment processes work and our eligibility criteria (that is which levels of need are given a particular priority).
