To use the service you must be assessed by Social Services to show that the service can meet your individual needs.
Adult Placement Schemes in Calderdale
The ‘Breakaway’ Scheme
This scheme is for you if you have learning disabilities. It offers you a chance to spend regular periods of time away from home, staying with another family. It provides you with the opportunity to make new friends, form new relationships and have new experiences. It may also help you to learn new skills and become more independent.
The ‘StayaWhile’ Scheme
This scheme could help you if you have, or have had, mental health problems. It can give you the chance to have a change of scenery and spend time away from home. Some carers offer a family environment, whilst some live alone and so can provide a quieter atmosphere with individual attention. Your carer will do their best to make you feel welcome and have an enjoyable stay.
The ‘Home from Home’ Scheme
If you are an older person this scheme could help you stay as a guest in the home of a family or single person. You could get out more and have a break from being by yourself. If you have a full time carer they could have a rest from their caring role whilst you are away.
Availability
All three schemes aim to be as flexible as possible and can provide both daytime and evening services, and overnight care.
The Long Term Scheme
There is a new, long term scheme for people with learning disabilities where they can live with families on long-term placements. This offers an alternative to living in a supported tenancy or residential home. It offers a person-centred way of living with consistent one-to-one support. It may also offer you the opportunity to become more independent.
To start using a service
If you’d like to use one of the above schemes you will need to have an assessment by Social Services to make sure the service can meet your individual needs. The assessment will also help to decide how often you can use the service. The person making the assessment will also complete a referral form giving some basic information about you and why you want to use the service.

Your service user plan will record any special requirements you have regarding dietary requirements, religious and cultural needs and preferred method of communication.
