Rapid response service / Intermediate Care Service
The service may be needed because you or your usual carers have a temporary illness or problem. The service could also provide you with rehabilitation support for a short time when you come out of hospital or to avoid acute hospital admission, so that you don't have to be re-admitted. The service can enable you to have a further assessment prior to long term care.
Support can be in the Community or a Rehabilitation Unit and will include both health and social care, from help with personal hygiene and care, domestic tasks, through to nursing and rehabilitation. The service aims to promote your recovery and independence. First of all we will make an assessment of your needs to see if the service is appropriate for you. If it is, we will provide a responsive package of support, care and/or treatment for you.
The Rapid Response / Intermediate Care Team
The Team comprises nurses, occupational therapists, physiotherapists, social worker, mental health practitioners, assistant practitioners, Rapid Response Team leader and assistants. In line with existing legislation, all team members have to pass a Criminal Records Bureau (CRB) check and receive a thorough induction. They are professionally qualified and the Rapid Response assistants will have, or be training for, National Vocational Qualifications (NVQ).
All team members are bound by rules of confidentiality and will not discuss your affairs with anyone else without your permission. Team members will always respect your privacy and dignity. They will not make sexist, racist, homophobic, ageist or other offensive remarks - and expect you to treat them respectfully in return.
The Rapid Response / Intermediate Care Service
- To be eligible for the service you must be aged 18 years or over, live in the Calderdale area, or registered with a Calderdale GP, and be medically stable or predictable as assessed by a doctor in the previous 24 hours.
- To use the service you must be referred to the Team by an appropriate person, such as a social worker, your GP, Hospital Wards, Accident and Emergency or a community nurse or therapist.
- There is no charge for this service.
- If you are receiving other services, then Rapid Response will be provided alongside any services you already receive.
- The Rapid Response Team works every day from 8.00 a.m. to 9.30 p.m., and outside these hours support is provided by out-of-hours teams from both health and social care.
- The service can be used for up to 14 days for the domicilliary service and up to 6 weeks in the Rehabilitation Units.
Complaints and compliments
It's good for team members to hear when you are particularly pleased with the service they give. You can tell the Rapid Response/Intermediate Care Team Manager on 01422 307354. The Team Manager can also help you to give a formal compliment if you wish to do this.
Your support should be right first time, but if not, then we need to know. We will apologise for any wrong on our part and put things right as quickly as possible. The Team Manager can also provide information about our formal complaints procedures.
