What the information will be used for
The information collected on the Register will help us to plan better services for disabled children. Personal details will be kept confidential within the Council and the NHS, but we may share anonymised statistical information with other organisations.
We keep the Register information separate from our other records.
The Register application is not a referral and the information will not be used to assess you for services.
People who can be placed on the Register
Your child can be included on the Register if s/he is under eighteen years old and has serious and lasting health problems or disabilities, including difficulties with moving, learning, seeing, hearing, or feeling, or with related behaviour that causes serious problems at home or at school.
Registration is not compulsory but we want to include as many children and young people as possible so that we can plan our services more effectively for the future.
You do not need to register your child in order to get help. If you feel that your child needs extra help you should talk to your child's GP, health visitor, teacher, social worker, community nurse or other worker who knows your child.

Each year we will ask you to update the information about your child on the Register. When your child reaches eighteen years of age we will ask your permission to pass on the information if your child transfers to be a user of Adult Services in Calderdale. If you want to take your child off the Register at any time, please inform the Register Co-ordinator.
