Moving to adult social care services
Young people who continue to need social care services when they reach age 18 will transfer to services provided by Adult Services. So, after the child’s 14th birthday, their Social Worker or Community Nurse will work closely with the Social Workers and Community Nurses in the Adult Services teams to make sure that they know about the child’s needs and that the transfer of service provider runs smoothly.
