Joining the library

Library membership form

To apply for membership of Calderdale Libraries, please complete this form. The form is comprised of sections, which are completed in separate parts.

• Part 1 - For all applicants, main details
• Part 2 - Details of a parent / carer for those under 16
• Part 3 - Help us provide the library services you need.

When you have completed the relevant sections, submit the form and you will be provided with a summary of the information you have sent.

Please note:

• All fields marked with an asterisk* are required and must be completed
• If you do not provide an email address it will be impossible for us to provide information to you by email
• It is not essential to provide a telephone number, but this will allow us to contact you quickly in the event of any problem with your library membership application.

The information you provide on this form will be only used by Calderdale Libraries Museums & Arts to operate its services, and will be stored securely as an electronic record. Your details will not be used for other purposes or be shared with third parties without your consent, unless there is a lawful reason to do so - for example, the disclosure is necessary to assist in the prevention or detection of crime or the apprehension or prosecution of offenders.
