Passport to Leisure

Applying for a Passport to Leisure

Applying for a Calderdale Passport to Leisure card couldn't be easier. Follow the instructions below and you will be able to get your card straight away.

1. Check 'Do I qualify for a Passport to Leisure?' to see if you qualify and have the relevant proof.
2. Either pick up a leaflet from one of the Registration Centres (see below) or download a form:

3. Complete all sections on the application form, read the Terms & Conditions and sign and date. (Please note, if you do not complete the necessary sections, you may not be issued with a card).
4. You will also need to take the following with you when you apply:
   
   • Proof that you qualify
   • Proof of your address (e.g. a household bill, bank statement etc)
   • A recent passport sized photograph
5. Take your completed form and the above items to any of the Registration Centres listed, where you will be issued with your card.
