Passport to Leisure
Applying for a Passport to Leisure
Follow the instructions below and you will be able to get your card straight away.
- Check 'Do I qualify for a Passport to Leisure?' to see if you qualify and have the relevant proof.
- Either pick up a leaflet from one of the Registration Centres or download a form: Passport to Leisure application form [PDF file 47KB]|
- Complete all sections on the application form, read the Terms and Conditions, sign and date it. (Please note, if you do not complete the necessary sections, you may not be issued with a card).
- You will also need to take the following with you when you apply:
- Proof that you qualify
- Proof of your address (eg a household bill, bank statement etc)
- A recent passport sized photograph.
- Take your completed form and the above items to any of the registration centres listed below, where you will be issued with your card.
